The Criminal Injuries Compensation Baord is trying to reach applicants who initiated claims but never returned the claim forms. From the CICB's website (http://www.cicb.gov.on.ca/en/alert.htm):

"Important Notice

The Board is attempting to contact applicants who initiated a claim after 1 July 1996, but did not return the claim forms that were mailed to them for completion.

If you did not submit your forms, and wish to pursue a claim with the Board, please contact our Claims Services Unit toll-free at 1-800-372-7463. The Claims Services Representative who receives your call will be happy to assist you.

If you received a letter informing you that forms are outstanding, and called the Board as a result, the appropriate action is being taken on your behalf. There is no need to re-contact the Board on this matter.

The Board is also contacting applicants who have a claim, which is missing information and is currently in process with a Compensation Analyst. If your file is in this category, you will receive a letter with information about what is outstanding and how to obtain these documents. If you require further assistance, please contact the Board and ask to speak to the Compensation Analyst noted in your letter."
